  What You'll Need:  
 
  • Your employer or depositor's name and address
  • Your employee ID or account number with depositor
  • Your account number
  • Your routing/ABA number
 
  What You'll Do:  
 
  1. Select the type of direct deposit you wish to set up or transfer and download the appropriate form:

    Type 1: Direct Deposit of Your Paycheck
    Check with your employer to verify that direct deposit is offered and then download and complete the appropriate direct deposit authorization form. Be aware that direct deposit may take up to three pay periods for processing.

    Type 2: Direct Deposit of Other Income
    Other types of income can be directly deposited into your account, such as funds from retirement or pension plans, dividends, interest, and income earned on investments. Download and complete the form below and mail it to the the appropriate organization.

    Type 3: Direct Deposit of Income from the U.S. Government
    Income such as Social Security or Military Pay is eligible for direct deposit. Access the standard government direct deposit form below and then visit your agency's website to determine where to mail your completed authorization.

  2. Print form and fill out in its entirety
    Your ABA/routing number and account number can be obtained from the voided check or view an ABA/routing number translation table.
